Falls Service
We work with people in the community aged 65 years or over, who have had a fall, or have been identified as being at risk of falls.

We are a therapy-led Falls Team comprising of Occupational Therapists, Physiotherapists and an Advanced Rehabilitation Assistant.
We complete a multifactorial assessment with our patients in order to:
- Identify the reason for the fall/ risk of falls
- Establish how falling has affected a persons day to day life and independence
- Identify how best to manage the risk of any future falls and how to cope in the event of a fall
- Identify any potential hazards within the home and how to make your home safer
- Review if current medication/medical history may be contributing to the fall
Following assessment, and working alongside you, we will set suitable goals and work towards these goals by developing a programme to:
- Improve your strength, balance and mobility through exercise at home
- Increase your activity levels and confidence, providing equipment when necessary, to maintain independence
- Help reduce potential hazards within the home
- Work with other services and agencies to further help reduce your risk of falls
This service operates Monday to Friday, 8am-4pm, for people in their own homes registered with a GP in the Dartford, Gravesend and Swanley area. For enquires or to make a referral, please contact us using the details above.
