The purpose of our service is to promote self management and independence by increasing patient knowledge regarding diabetes and its potential long term complications.

We work closely with your GP/Practice Nurse and consultant to ensure that we provide a comprehensive service, to enable you the patient to live an independent life.

Type 2 diabetes – X-PERT education this education programme involves attending 6 x 2½ hour sessions, held over 6 consecutive weeks at Sheppey Community Hospital or Sittingbourne Memorial Hospital. We offer either morning, afternoon or evening courses, with a six month and 1 year follow up. This course gives you the opportunity to meet other people with Type 2 diabetes and to learn about how this long term condition may affect you, and how you may wish to make changes to your lifestyle and diet in order to slow the progression of diabetes.

Type 1 diabetes—T1DE (Type 1 Diabetes Education) this education involves attending 4 whole days, over 4 consecutive weeks. Individuals need to be administering insulin via a basal bolus ( 4 x daily insulin) regimen in order to attend this programme. Participants will be taught  how to adjust their insulin doses in conjunction with their carbohydrate intake and activity levels.

Home visits

These are available for patients who are housebound, either temporarily, or permanently. If you are able to visit your GP practice then it will be expected that you would be able to attend a clinic appointment/venue
