Harty Ward

Harty Ward is an inpatient ward within Sheppey Community Hospital. The ward is over seen by a team of, registered nurses, physiotherapists, occupational therapists, a pharmacist health care assistants, rehabilitation assistants, a care manager and a visiting General Practitioner.
We provide a rehabilitation-focused inpatient approach to care. There are 24 beds available and the hospital takes patients following referral either directly from their own homes or from an acute hospital. We are able to provide end of life and specialist stroke care.
Our aim is to help patients regain their independence where appropriate, and to teach them coping strategies, for example after a stroke. Staff work in partnership with the patient, carer and relatives to ensure all aspects of care, including assessment of needs on admission, joint goal setting, planning, implementation and evaluation of care, are effective.

Kestrel Ward is an in patient ward at Sittingbourne Memorial Hospital. It is over seen by a team of , registered nurses, physiotherapists, occupational therapists, a pharmacist, healthcare assistants, rehabilitation assistants a care manager, and a visiting General Practitioner.
We provide a rehabilitation-focused inpatient approach to care. There are 23 beds available and the hospital takes patients following referral either directly from their own homes or from an acute hospital. We are also able to provide end of life care.

Livingstone Ward

The ward is over seen by a team of registered nurses, physiotherapists, occupational therapists, a pharmacist health care assistant, rehabilitation assistants, a care manager and a doctor.
We provide a rehabilitation-focused inpatient approach to care. There are 24 beds available and the hospital takes patients aged over 18 following referral either directly from their own homes or from an acute hospital. We provide care to the frail older person and we are able to provide post orthopaedic care e.g. a new hip or knee and have been referred to the service by the acute local hospital or patients who require general rehabilitation following a fall.
Our aim is to help patients regain their independence in the aim of returning home, and to teach them coping strategies. Staff work in partnership with the patient, carer and relatives to ensure all aspects of care, including assessment of needs on admission, joint goal setting, planning, implementation and evaluation of care, are effective.
