Medway Intermediate Care and Reablement
The Medway Intermediate Care and Reablement Service supports adults recovering from a hospital stay due to an illness or injury to regain their independence and remain safely at home. We provide short-term rehabilitation, therapy and practical support through personalised care plans focused on helping people achieve their recovery goals. Working closely with health, social care and community partners, we help people rebuild confidence, develop everyday living skills and avoid unnecessary hospital stays wherever possible. About the team
The Intermediate Care and Reablement Service supports adults who need short-term rehabilitation following a hospital stay due to illness or injury. The teams aim is to help people recover, regain confidence and maintain their independence for as long as possible.
We are a multidisciplinary team that includes occupational therapists, physiotherapists, rehabilitation and enabler staff as well as administrative colleagues. Working together with service users, families and carers, we develop personalised care plans focused on individual goals and recovery needs.
Our approach is centred on reablement, which means helping people do as much as possible for themselves. We support people to build confidence and develop or regain everyday living skills such as washing, dressing, preparing meals, moving around safely and managing daily routines.
Support is usually provided in a person's own home, where many people recover best. For those who need a greater level of rehabilitation or are not yet at a recovery stage where home would be a safe place, support may include a short stay in a community intermediate care bed before returning home.
We work closely with local health, social care and voluntary sector partners to ensure people receive coordinated support throughout their recovery. Progress is reviewed throughout their journey with weekly structured service users/team review meetings, and when support from our service is ending, we help people access any ongoing care or community services they may need. The service forms part of a joined-up approach to intermediate care across Medway, making it easier for people to receive the right support at the right time.
Our goal is to help people achieve the highest possible level of independence and live safely and well in their own home.
Who this service is for
We can usually help if:
- You are recovering from recent hospital stay due to an illness or jury and need short-term support to regain your independence
- You have experienced a change in your physical abilities and would benefit from rehabilitation or reablement
- You need support to rebuild confidence with everyday activities such as washing, dressing, preparing meals or moving around safely
- You would benefit from occupational therapy, physiotherapy or rehabilitation support to achieve agreed recovery goals
- You need support following a discharge from hospital to help you recover safely at home
- You live in Medway (postcodes ME1 to and including ME8)
We may not be the right service if:
- You require urgent medical treatment that needs hospital admission
- You require long-term care and support without a rehabilitation or recovery goal
- You require a permanent residential or nursing home placement
- Your needs would be more appropriately met by a specialist community healthcare service
- You are already independent and do not require rehabilitation, reablement or recovery support
