Referral Guidance for Professionals
North Kent Adult Community Health Services operates a Single Point of Access (SPA) and Care Coordination Centre, providing a streamlined route for healthcare professionals to refer patients into our community services.
Referrals can be made using the referral process outlined below, ensuring patients are directed to the most appropriate service for their needs. Our team supports access to a wide range of adult community health services across Dartford, Gravesham, Swanley, and Swale, helping patients receive the right care in the right place at the right time.

About this service
Healthcare professionals can refer patients into any of our Adult Community Health Services using our single referral form, available below. This approach provides a simple and consistent referral process across all community services, helping to ensure referrals are reviewed promptly and directed to the most appropriate team.
For all new referrals, please complete the referral form and submit it via email to our Single Point of Access (SPA) and Care Coordination Centre. Our team will review the information provided and coordinate access to the most suitable service based on the patient's needs.
If you wish to discuss, update, or seek advice regarding a patient who is already known to our services, please contact the Single Point of Access by telephone. In these circumstances, a new referral form is not required. The referral form should only be completed for a patient's first referral into our services.
The Single Point of Access acts as a central coordination hub for referrals into North Kent Adult Community Health Services. This helps reduce duplication, improves communication between services, and ensures patients can access the support they need as efficiently as possible.

Who this service is for
We can usually help patients who are:
- Being referred by a healthcare professional
- Aged 18 years or over
- Living within the North Kent areas of Dartford, Gravesham, Swanley, or Swale, or are registered with a GP in these localities
- Having a health need that can be safely managed within a community-based healthcare setting
- Requiring assessment, treatment, rehabilitation, monitoring, or support from one of our community health services
- Referred through our Single Point of Access and Care Coordination Centre
We may not be the right service for patients if:
- They require emergency or life-threatening care In these situations, call 999 or direct the patient to the nearest A&E
- The referral relates to a service that sits outside the scope of North Kent Adult Community Health Services
