Care Coordination Centre

The Care Coordination Centre provides a single point of access for all telephone enquiries and referrals relating to commissioned community services, excluding community inpatient wards.

About the team

Our administrative team ensures that all contacts are managed efficiently and directed to the most appropriate clinical team to support timely, safe and effective care.

All enquiries and referrals are received, triaged and routed through a centralised access function. This model provides callers and referrers with a clear and consistent point of contact and supports seamless navigation between services.

The service operates seven days per week from 8:00am to 8:00pm, including weekends and Bank Holidays. Outside of these hours, all calls are automatically redirected to the Out of Hours Nursing Service to ensure continuity of access.

  • You need to be directed to the most appropriate team for support

We may not be the right service if:

  • You need emergency medical help. Call 999 or go to A&E
  • You already know which healthcare service you need and can contact them directly
  • You need ongoing treatment from a specific clinical team
  • Your enquiry is not related to local healthcare services
