Community and Night Nursing
The Community Nursing Service provides nursing care for people who are unable to leave their homes either longer term because of illness, disability, end of life, frailty or as a result of a short spell of illness or surgery which prevents them from attending their GP practice for their health needs to be met. The service supports people to manage their health, recover from illness and receive treatment safely in their own home. Care is available both during the day and overnight when needed.

About the team
The service is delivered by district nurses, staff nurses, healthcare support workers and care coordinators working as part of local neighbourhood teams and operates 365 days a year 24 hours a day. The team provides assessment, treatments such as wound care, medication support, long-term condition management and end of life care. Overnight nursing support is provided through the Night Nursing Service.
Who this service is for
We can usually help if:
- You are an adult who requires nursing intervention in your own home
- You are an adult unable to attend clinic appointments because of illness, disability or frailty
We may not be the right service if:
- You are able to access appropriate nursing interventions through their GP surgery, outpatient or clinic services.
