Community Cardiology and Heart Failure
The Community Cardiology and Heart Failure Service supports people living with heart failure and other heart conditions. The service helps people manage their symptoms, stay as well as possible and reduce the risk of unnecessary hospital admissions through specialist assessment, monitoring and treatment.

About the team
The team includes specialist cardiac nurses, therapists and support staff who work closely with GPs, hospital cardiology teams and other community services. Support may include heart failure management, cardiac rehabilitation, diagnostic testing, education and monitoring of long-term heart conditions. The service operates Monday to Friday from 8:00am to 4:00pm excluding Bank Holidays.

We can usually help if:
- You live with heart failure
- You are diagnosed heart conditions requiring specialist support
- You require cardiac rehabilitation or diagnostic assessment
We may not be the right service if:
- You require emergency hospital treatment for an acute cardiac event
