Community Virtual Ward

The Community Virtual Ward supports the delivery of clinical care and monitoring to individuals within their usual place of residence. Individuals are clinically assessed to ensure their needs can be safely met in the home environments and not in an acute hospital bed.

About the team

The service ensures patient reviews which could be at multiple points in a day, depending on need and can be a mixture of face-to-face or virtual. The service utilises digital technology to support remote monitoring. It can also support people leaving hospital early but still require ongoing monitoring and treatment.

The team consists of advanced nurse practitioners and other community clinicians working closely with GP's, hospital consultants and specialist teams. Patients are monitored remotely and receive regular clinical support, with arrangements in place to respond quickly if their condition changes. They operate 365 days a year from 8:00am to 8:00pm.

Who this service is for

We can usually help if:

  • You require ongoing monitoring and treatment that would otherwise need a hospital stay.
  • Your clinical needs can be safely managed at home with appropriate support

We may not be the right service if:

  • Your condition is unstable or at an acute level that requires hospital emergency department review and continuous hospital supervision.

Questions and answers

What is a virtual ward?
A virtual ward provides hospital-level monitoring and treatment while you remain at home. You are still under the care of healthcare professionals, even though you are not staying in hospital.
What happens if my condition gets worse?
Your healthcare team will regularly monitor your condition; you will have a clear developed care plan with escalation factors and contact details. If there are concerns about your health, hospital support/guidance will be sought. However, as this service works alongside the hospital teams, any deterioration would be identified and managed appropriately.
