Multidisciplinary Coordinators

The Multidisciplinary Coordinators (MDT) Service supports people with complex health and care needs to ensure they have access to the required services in a timely manner.

About the team

The MDT coordinators perform a key function within the Integrated Neighbourhood Health (INT) and ensure that the required health and required health and care partners are present at the MDT meetings to make sure support is joined up, well organised and focused on the needs of the individual.
The aim is to help you stay as independent as possible and receive the right care at the right time and the right place.

They achieve this by working closely with GPs, community health services, social care teams and voluntary organisations. The service operates Monday to Friday from 9:00am to 5:00pm excluding Bank Holidays.

They help coordinate care planning, arrange reviews and make it easier for professionals to work together. This helps reduce delays, avoid duplication and ensure everyone involved in your care has the information they need.

Who this service is for

We can usually help if:

  • You have a complex health or social care need
  • You are an individual supporting this population group where they believe support from multiple services is required
  • You would benefit from coordinated care planning and review

We may not be the right service if:

  • Your needs can be met by a single service without requiring care coordination support
